BackgroundBioethics as a field related to the health system and health service delivery has grown in the second half of the 20th century, mainly in North America. This is attributed, the author argues, to mainly three kinds of development that took place in the developed countries at a pace different than the developing countries. They are namely: development of the health system; moral development; and political development.DiscussionThis article discusses the factors that impede the development of the field of bioethics from (...) an academic activity to a living field that is known and practiced by the people in the developing countries. They are quite many; however, the emphasis here is on role of the political structure in the developing countries and how it negatively affects the development of bioethics. It presents an argument that if bioethics is to grow within the system of health service, it should be accompanied by a parallel changes in the political mindsets in these countries.SummaryFor bioethics to flourish in developing countries, it needs an atmosphere of freedom where people can practice free moral reasoning and have full potential to take their life decisions by themselves. Moreover, bioethics could be a tool for political change through the empowerment of people, especially the vulnerable.To achieve that, the article is proposing a practical framework for facilitating the development of the field of bioethics in the developing countries. The provision of health care service in resource-poor settings is associated with a broad set of ethical issues. Devakumar's case discusses the ethical issues related to the inability to treat in a cholera clinic patients who do not have cholera. This paper gives a closer look on the context in which Devakumar's case took place. It also analyses the potential local and organizational factors that gives rise to ethical dilemmas and aggravate them. It also proposes a framework to help in (...) the proactive handling of the factors that leads to ethical dilemmas and resolving the ethical issues as they appear. It adopts the four principles of autonomy, beneficence, non-maleficence and justice as universal and prima facie principles, but with the inclusion of a local understanding of what of each of these principles means. It is based on a collaborative approach that involves the beneficiaries and other partners in the field to help share information and resources, as well as adopting the provision of a wider service to the whole community. This is done by asking three basic questions: (a) who are the relevant stakeholders? (b) what ought to be the ethical principles in place? and (c) how should we take, implement and follow the decision about service provision? (shrink)

I had been here before, although I wore a different hat as I literally walked under the burning April sun along the wide, dusty, unpaved streets of Nyala, South Darfur, to “headquarters.” It was to be another interesting, but normal, peaceful, and safe day as I led the Sudanese household survey in (...) South Darfur, then the largest of all Darfur states. That was in 2006, a few years after I joined the Federal Ministry of Health. My office overlooked the Nile, but getting out of the capital city of Khartoum was always the best part of working there. I had the opportunity to meet real people in their real places, where they are home and at ease.Overall, the data collection team’s work went according to plan. Most of the randomly selected villages were visited as planned or replaced according to protocol. But this morning was anything but normal. A few days earlier, while visiting a village in a nearby town, Kass, my team was able to make a single phone call to us, the leadership [End Page 162] team, composed of two colleagues from Darfur and myself, indicating the team had no reception—the nearest signal was an hour’s drive away. The team leader assured me that they had started collecting data and promised to call every day or every other day. They were a team of six young men and women (I called them boys and girls because they were so young and innocent) between the ages of 20 and 25, all from Darfur. Two of them were in love and participated in the survey collection to save the per diem for their wedding.The time I was expecting their call came, but the call did not come. Then another day, then another, then another.One of the management team was in charge of the security update, and that morning he asked me in a shaky voice if we could have a private meeting as soon as I entered the room. He informed me that the village where my team was located had been attacked by the Janjaweed, the notorious militia known for its ruthless, brutal attacks that leave nothing behind, but men killed indiscriminately, houses and children on fire, and women raped. I recalled my experience in Darfur the year before when I mechanically asked a woman if she had lost a family member in the last six months and what the cause of death was. She burst into tears. I tried to calm her down. Finally, she told me that her few-month-old baby had been burned alive in front of her own eyes. Before I could even imagine the fate of the boys and girls on the team, a call came in. It was not from the team, but from a crying mother who asked me to “bring her daughter back.” She told me she did not want her daughter killed or raped. I cannot remember what I said because I was in shock and did not want anyone to see this, especially the other teams. The last thing I needed was to see the panic for which I was not trained to manage.I had no time to waste. I had no exit or contingency plan in the 224-page survey guide that explained how to collect the data, how to ask the questions, how to sort and safely store the paper questionnaires, how to enter the data. It seemed to provide extensive guidance on everything except for the only instruction I needed at the time—How to keep my teams safe under fire. I took the first flight to the survey’s national headquarters in Khartoum seeking help. I was promised to be put in contact with the intelligence and army in the region through the government agency responsible for organizing humanitarian work in the country. The discovery of clustered regularly interspaced short palindromic repeats (CRISPR) and the CRISPR-mediated protein 9 (CRISPR-Cas9) immediately revealed numerous potential therapeutic applications. Although CRISPR-Cas9 will most likely be useful for addressing issues such as genetic diseases and related medical issues, use of this modality for germline modification generates complex ethical questions regarding the safety and efficacy, human genetic enhancement, and “designer” babies. In this article, the case of the He Jiankui affair is used as an example of the potential for (...) unregulated use of CRISPR-Cas9 technology. In 2018, Dr He Jiankui reported that he had successfully edited human embryos. This work clearly violates all international principles of bioethics. As such, the purpose of this paper is to explore the ethical challenges inherent in the use of CRISPR-Cas9 for human germline editing from the perspectives of the goals of Islamic law (Maqasid al Shari'a) and the major jurisprudential maxims (Qawaid Fiqhiyyah). We argue that from an Islamic standpoint, the therapeutic application of CRISPR-Cas9 for germline editing may be permissible if the safety and efficacy concerns are resolved and if the principles of Maqasid al-Shari'a are fulfilled. Armed conflict in Darfur, west Sudan since 2003 has led to the influx of about 100 international humanitarian UN and non-governmental organizations to help the affected population. Many of their humanitarian interventions included the collection of human personal data and/or biosamples, and these activities are often associated with ethical issues. A systematic review was conducted to assess the proportion of publicly available online reports of the research activities undertaken on humans in Darfur between 2004 and 2012 that mention obtaining ethical (...) approval and/or informed consent. This systematic review is based on a systematic literature search of Complex Emergency Database, ReliefWeb, PubMed), followed by a hand search for the hardcopies of the eligible reports archived in the Centre for Research on the Epidemiology of Disasters in Brussels. The online search showed that out of the 68 eligible studies, 13.2% reported gaining ethical approval and 42.6% that an informed consent was obtained from the participants. The CRED search included 138 eligible reports. None of these reports mentioned gaining ethical approval and 17 mentioned obtaining informed consent from their participants. The proportion of studies reporting ethical review and informed consent was smaller than might be expected, so we suggest five possible explanations for these findings. This review provides empirical evidence that can help in planning ethical conduct of research in humanitarian settings. (shrink)

The application of recycled aggregate as a sustainable material in construction projects is considered a promising approach to decrease the carbon footprint of concrete structures. Prediction of compressive strength of environmentally friendly concrete containing recycled aggregate is important for understanding sustainable structures’ concrete behaviour. In this research, the capability of the deep learning neural network approach is examined on the simulation of CS of EF concrete. The developed approach is compared to the well-known artificial intelligence approaches named multivariate adaptive regression (...) spline, extreme learning machines, and random forests. The dataset was divided into three scenarios 70%-30%, 80%-20%, and 90%-10% for training/testing to explore the impact of data division percentage on the capacity of the developed AI model. Extreme gradient boosting was integrated with the developed AI models to select the influencing variables on the CS prediction. Several statistical measures and graphical methods were generated to evaluate the efficiency of the presented models. In this regard, the results confirmed that the DLNN model attained the highest value of prediction performance with minimal root mean squared error. The study revealed that the highest prediction performance could be attained by increasing the number of variables in the prediction problem and using 90%-10% data division. The results demonstrated the robustness of the DLNN model over the other AI models in handling the complex behaviour of concrete. Due to the high accuracy of the DLNN model, the developed method can be used as a practical approach for future use of CS prediction of EF concrete. (shrink)

The 3D Prandtl fluid flow through a bidirectional extending surface is analytically investigated. Cattaneo–Christov fluid model is employed to govern the heat and mass flux during fluid motion. The Prandtl fluid motion is mathematically modeled using the law of conservations of mass, momentum, and energy. The set of coupled nonlinear PDEs is converted to ODEs by employing appropriate similarity relations. The system of coupled ODEs is analytically solved using the well-established mathematical technique of HAM. The impacts of various physical parameters (...) over the fluid state variables are investigated by displaying their corresponding plots. The augmenting Prandtl parameter enhances the fluid velocity and reduces the temperature and concentration of the fluid. The momentum boundary layer boosts while the thermal boundary layer mitigates with the rising elastic parameter strength. Furthermore, the enhancing thermal relaxation parameter ) reduces the temperature distribution, whereas the augmenting concentration parameter drops the strength of the concentration profile. The increasing Prandtl parameter declines the fluid temperature while the augmenting Schmidt number drops the fluid concentration. The comparison of the HAM technique with the numerical solution shows an excellent agreement and hence ascertains the accuracy of the applied analytical technique. This work finds applications in numerous fields involving the flow of non-Newtonian fluids. (shrink)

The Holy Qur’an relates believers’ pure emotions to brotherhood. From the perspective of Islam, it has been also affirmed that two people have the same thoughts and opinions in one centre point of love in brotherhood, so fraternal feelings are not merely limited to kinship and consanguinity. In this line, the Holy Qur’an states that we have come to make you brothers with each other; not a brotherhood from parents, but an Islamic one, which is highly appreciated in the Islamic (...) society. In addition, a believing brother can never be filled when his fellow believer is hungry, and he cannot be clothed while the other is with nothing on. Islam further considers all Muslims as one family and calls everyone brothers and sisters or siblings. The true Muslims of any race and tribe as well as any language and age thus have a deep sense of brotherhood with each other, even if one lives in the Eastern parts of the world and the other in the West. In sum, this article aimed to investigate the role of religious teachings in strengthening believers’ brotherhood in the Islamic society. For this purpose, firstly, the definition of religious brotherhood was provided; secondly, religious brotherhood has been described based on the Holy Qur’an.Contribution: This article helped gain a new insight into the role of Islamic teachings in strengthening religious brotherhood in the Islamic society. Besides, the indicators of religious brotherhood were extracted and then enlightened according to the Ayat (viz. verses) from the Holy Qur’an. (shrink)

BackgroundMonitoring recovery–stress balance in sport is becoming more relevant to prevent training maladaptation and reach the optimal performance for each athlete. The use of questionnaires that identify the athlete’s recovery–stress state have much acceptance in sports due to reliability and useful, furthermore for its low cost. Identifying possible differences between sport modalities and sex is important to determine specific needs and possible intervention ways to keep a recovery–stress balance. The aim was to analyze the differences in the recovery–stress state and (...) mood states by sex and sport type during the competitive phase in young Mexican athletes. As a secondary objective, the psychometric properties of the Mexican version of the Recovery–Stress Questionnaire for Athletes were analyzed.MethodsA cross-sectional study was carried on with 461 athletes, 17.95 years old, from six sports disciplines. The RESTQ-Sport and Profile of Mood States were applied in a single moment. Differences by sex and sports modality were analyzed. RESTQ-Sport’s confirmatory factor analysis was performed after the stress and recovery theoretical structure of two stress and two recovery dimensions, and last, the concurrent validation with the POMS was carried on.ResultsSignificant differences by sex were found in the General Recovery and Sport Stress dimensions of the RESTQ-Sport as well as Vigor factor of the POMS, being higher for men; furthermore, both the Sport Recovery dimension of RESTQ-Sport and Cholera and the Fatigue and Depression factors from POMS also had differences by sport type, showing a less recovery and high stress for individual sport athletes. Goodness-of-fit indexes of the model for the RESTQ-Sport were acceptable. Pearson’s correlation between questionnaires was moderate.ConclusionThe recovery–stress state shows differences in the function of sex and sport modality. More special attention is suggested for women and individual sport athletes. The higher punctuation for men compared with women in sport stress dimension did not negatively affect the recovery–stress balance for male athletes. Finally, the Mexican context adaptation of the RESTQ-Sport provides a psychometric instrument suitable to assess the recovery–stress balance in Mexican athletes. (shrink)

When people engage in conversation, they tailor their utterances to their conversational partners, whether these partners are other humans or computational systems. This tailoring, or adaptation to the partner takes place in all facets of human language use, and is based on a mental model or a user model of the conversational partner. Such adaptation has been shown to improve listeners' comprehension, their satisfaction with an interactive system, the efficiency with which they execute conversational tasks, and the likelihood of achieving (...) higher level goals such as changing the listener's beliefs and attitudes. We focus on one aspect of adaptation, namely the tailoring of the content of dialogue system utterances for the higher level processes of persuasion, argumentation and advice-giving. Our hypothesis is that algorithms that adapt content for these processes, according to a user model, will improve the usability, efficiency, and effectiveness of dialogue systems. We describe a multimodal dialogue system and algorithms for adaptive content selection based on multi-attribute decision theory. We demonstrate experimentally the improved efficacy of system responses through the use of user models to both tailor the content of system utterances and to manipulate their conciseness. (shrink)

The reality of revelation was one of the fundamental questions that\noccupied George Tyrrell as a writer until he died on 15 July 1909. The\ncentenary of his death is an opportune time to engage this English\nModernist in a dialogue with Karl Rahner on the subject of revelation.\nTyrrell insists on the primacy of the interior experience of revelation.\nAn exaggerated emphasis on inner religious experience, however, led him\ninevitably to a separation of the interior dimension of revelation from\nits verbal expressions and doctrinal formulations.\nRahner also (...) affirms the primacy of the originating inner experience of\nGod but stresses at the same time the intrinsic unity between this\ntranscendental revelation and its categorical, historical dimension.\nRevelation corresponds to the symbolic nature of the addressee. The\nMystery of the Incarnation is the point of reference for understanding\nGod's self-communication. The fullness of revelation has been realized\nin the indissoluble and irreversible unity of the Divine Logos with the\nMan Jesus. (shrink)

Two major functions of the visual system are discussed and contrasted. One function of vision is the creation of an internal model or percept of the external world. Most research in object perception has concentrated on this aspect of vision. Vision also guides the control of object-directed action. In the latter case, vision directs our actions with respect to the world by transforming visual inputs into appropriate motor outputs. We argue that separate, but interactive, visual systems have evolved for the (...) perception of objects on the one hand and the control of actions directed at those objects on the other. This 'duplex' approach to high-level vision suggests that Marrian or 'reconstructive' approaches and Gibsonian or 'purposive-animate-behaviorist' approaches need not be seen as mutually exclusive, but rather as complementary in their emphases on different aspects of visual function. The vast majority of contributions to the literature on spirituality in nursing make extravagant claims about transcendence, eternity, the numinous, higher powers, higher levels of existence, invisible forces, cosmic unity, the essence of humanity, or other supernatural concepts. Typically, these assertions are made without the support of argument or evidence; and, as a consequence, alternative ways of theorizing ‘spirituality’ have been closed off, while the lack of consistent scholarship has turned the topic into a metaphysical backwater. In this paper, I (...) adopt a different premise, rejecting unsupported claims, and surveying the ‘spirituality’ terrain from a naturalistic and reductionist perspective. I argue that, if we rid ourselves of non‐naturalistic assumptions, we will discover theoretical and clinical resources in health psychology, social psychology, neuropsychology, and pharmacopsychology – resources which confirm that it is possible to conceptualize the study of existential concerns, and how health professionals might respond to them, in a properly scientific manner. In order to illustrate the potential usefulness of these resources, I will take palliative care as an example. (shrink)
